How it works
- A patient submits your Jotform intake form
- Keragon searches ModMed for an existing patient record using the form data
- If the patient is found, their ModMed profile is updated with the latest form information
- If no match exists, a new patient is created in ModMed
- The sync runs instantly for every submission — no staff action needed
